The eternity band looks like you had a layer failure and it didn't get picked up again until you got to the top part of the band.
My first guesses might be debris interference during the print or the exposure needs to be slightly adjusted higher.

From the photo, is it a positive defect or a negative defect? Positive defect would be a investment failure, while a negative defect can be lots of things, many of which you mention. Each input on a projector may have different settings in the configuration. Did you check to see if your VGA/Computer and HDMI settings are different?

I bought a small one to try to make metal shot from my scrap silver and after the first time I fired it with a torch, it started to disintegrate. Bought it on eBay so it isn't top of the line, but it really surprised me.

Do you have a photo to post of the undersized ring after it is printed? That might give us some additional insight.

Some other ideas could be some stuck pixels in the projector or possibly a stray reflection of light back into the vat causing partial exposure in that region?

The print shifted completely, which implies the either the print shifted when peeled, the image from the projector shifted pixel-wise during the print or the build platform moved and then stayed in that position.
